Submit Exam Information

To request Academic Testing Services for your course(s), instructors must submit exam information each semester.

Service Level Agreement (SLA)

Before submitting the exam information request form, review the Service Level Agreement, which establishes a clear and mutually beneficial understanding between NC State University instructors and Academic Testing Services (ATS). This agreement outlines the terms, responsibilities, and procedures that ensure the efficient and effective administration of exams.

Priority for Testing Services

Testing Services will prioritize requests for proctoring based on course type and instructional needs. The order of priority is as follows:

  1. Fully Internet-Based Classes: Classes without an assigned classroom.
  2. On-Campus Students with Testing Accommodations: Students whose DRO-approved accommodations cannot be met in the classroom environment.
  3. Hybrid Classes: Classes where the scheduled in-person meeting time is less than the total credit hours.
  4. On-Campus Classes: Classes with assigned classrooms and enrollments of over 75 students.

Exam Information Form Submission

Full Class Testing Request

  • The submission deadline is the fifth day of classes each semester.
  • separate form must be completed for each class section.
  • Submitting the form does not guarantee that ATS will be able to proctor your exams. Requests are subject to test center availability, course priority, and existing accommodated testing demands.
  • Instructors will receive an email confirmation at their NC State University email address if in-person proctoring is approved for their course.

Accommodated Testing Requests

  • Instructors should submit the Exam Information Form by the fifth day of classes, and no later than 10 days before the exam date.
    • Students cannot schedule appointments until the completed form, including all exam details, has been submitted.
    • Instructors must include ALL semester exam dates, allowed materials, and the standard (unextended) time limit for the exam.
      • The exam PDF does not need to be uploaded upon form completion
  • Instructors with paper exams must submit the material as a single file, preferably in PDF format, no less than three business days before the exam start date to avoid appointment cancellation.
  • Exam files must include any required format accommodations (e.g., enlarged print, screen-reader accessible versions).

Important Information before completing the request form

Exam Windows

  • Full-Class Testing requires a window based on the matrix below and a minimum of three days during the final exam period.
  • On-campus accommodated testing may be scheduled within a one-day exam window. However, Testing Services will not guarantee that exam appointments will overlap with the scheduled class time.
  • Exam windows must include full business days (not partial days).

During the Fall and Spring Semesters, full class courses should follow the recommended minimum exam windows.

Exam Time Limits

  • To support efficient scheduling and equitable access for all students, instructors should select exam time limits that reflect the actual time needed for completion.
  • Standard Time Limits: 60, 90, 120, 150 minutes
  • Accommodated Testing
    • When submitting exam information, instructors should always provide the standard (unextended) exam time limit. Testing Services will apply the approved extended time accommodations as needed by the individual student.

Note: Instructors are strongly encouraged to consider alternative assessments (e.g., projects, open-book assignments) in place of proctored exams shorter than 60 minutes, as these can be inefficient to schedule and administer.

Allowed Materials

  • Only materials listed on the Exam Information Form will be allowed in the testing room.
  • All permitted materials—including notes, formula sheets, and scrap paper—will be collected at the end of the exam and securely destroyed.
    • Students must make any necessary copies of their notes before arriving at the test center.
  • Personal electronic devices (e.g., laptops, tablets, smartwatches, phones) are not permitted.
    • Students with electronic notes should be able to access them via their Learning Management System.
  • Calculators with storage or internet access (e.g., TI-Nspire) are not permitted.
