Submit Exam Information

To request DELTA Testing Services for your course(s), instructors must submit exam information once a semester for each course. A separate form is required for each class section. The deadline for exam submissions is the fifth day of classes.

Service Level Agreement (SLA)

Review the Service Level Agreement and the selection and prioritization of classes to receive in-person proctoring at the test center. All requests to schedule exams are subject to test center availability. 

Table 1 shows required exam windows based on student enrollment and exam time limit. Test dates are available on a first-come basis. If a date you desire is unavailable, the test center is either closed or at max capacity. Choose the next available date or consider altering your exam window.

Table 2 shows the limitations of certain allowed materials and alternative suggestions based on best practices. The test center will assume note pages can be front and back (duplex) unless otherwise specified in the “special instructions” section. Notes allowed will be collected at the end of the exam. Students should make a copy of their notes prior to arriving at the test center.
