Submit Exam Information for Accommodated Testing

If an instructor cannot set up testing accommodations within the department, test proctoring services can be set up through Accommodated Testing Services. Accommodated Testing Services will provide students with a location to take accommodated exams in a professional, secure, and monitored setting.

Review the Service Level Agreement for Accommodated Testing Services outlining the terms of service required to efficiently and effectively administer exams.

Instructors who would like to request Accommodated Testing Services to assist with proctoring accommodated exams must:

  • Complete the exam information form at the beginning of the semester or at least 10 days before the exam date.
    • Students can not schedule appointments until the exam information form, including all exam details, is completed. 
      • The submission of the exam information form does not in itself guarantee that Accommodated Testing Services will proctor your exams. All exam scheduling requests are contingent upon test center availability and exam requirements.
    • Submission of the form will authorize all students with accommodations to schedule appointments.
  • Submit ALL the exam dates, allowed materials, and time limit (without extended time accommodation).
    • Make sure to include any format accommodations (enlarged print, accessible to a screen reader, etc.).
  • Submit a PDF copy of the exam material. If a PDF copy of the exam material is not provided upon completion of the exam information request form, please ensure that a PDF copy of the exam is submitted to dro-testing@ncsu.edu no later than three business days before the exam date.
    • Student appointments will be canceled if the exam is not received within three business days of the exam date and the instructor will be responsible for administering the exam.

An email will be sent to your NC State University email address if in-person exam proctoring for your course is confirmed.
